Required Documents
These 2 documents are REQUIRED by insurance. We must obtain BOTH of these documents from the client and submit them to insurance BEFORE we can begin therapy.
1. A prescription for ABA therapy that MUST specifically say, "ABA" on it in order for insurance to accept it. This must be signed by either a MD, DO or PhD (no nurse practitioners, social workers etc.).
2. A diagnostic report which is a 10-15 page report that explains the testing that was completed and scores obtained in order to arrive at the diagnosis(es). This must be signed by either a MD, DO or PhD (no nurse practitioners, social workers etc.).
Submission
By Fax
Ask your doctor to fax both the prescription for ABA and the diagnostic report to us at 833.379.4405. BOTH documents MUST be received by us in order to be added to our waiting list. If only one document is received it will be deleted.
